What Is Hallucination?

A hallucination is AI output that is fluent, confident, and wrong: invented court cases, fabricated citations, plausible-sounding statistics with no source, biographical details that belong to someone else. It happens because language models generate what is statistically plausible, not what is verified true. When the training data thins out around a question, the model does not feel uncertain the way you do; it simply continues with the most plausible-sounding text, and plausible is not the same as real.

The failure mode has consequences: lawyers have been sanctioned for filing briefs citing nonexistent cases, and every profession adopting AI has its cautionary tale. Rates have fallen substantially with each model generation, and grounding techniques (see RAG) cut them further by forcing answers to come from supplied documents. But no current system eliminates them, and anyone who tells you otherwise is selling something.

The defensive playbook is short and non-negotiable. Verify anything that must be true: names, numbers, quotes, citations, laws, medical claims. Prefer grounded tools (with citations you actually click) for research. Ask the model for its sources and confidence, useful, though not sufficient. And structure high-stakes workflows so a human reviews before anything ships; every AI disaster story skipped exactly that step.

The mental model that serves best: treat AI like a brilliant, fast, occasionally overconfident junior colleague. Wonderful first drafts, mandatory review.
